Visit during office hours so our staff can learn what happened and determine whether CHORC may be able to help.

After the initial conversation, we will explain the forms and supporting documents required for your situation.

Attend your appointment

Once all documents are received, CHORC schedules an assistance appointment and completes the eligibility review.
